Recommendation Form
Three reference forms are required; two from science instructors and one from an academic or a personal reference who knows you well. The online application will ask for name and contact information for your three references. Recommenders may fax 252-744-3015 or e-mail their reference letters to: SPFD@ecu.edu
AMCAS Release Form
(for students who have previously applied to medical school)
This release form gives our office permission to access copies of your AMCAS application, transcripts, and essay. Please complete and submit with your application. Letters of recommendation cannot be released.
