The MOTHeRS Project framework was created using the NC-STeP model in hopes to bring a team-based, patient-centered collaboration of specialists to the primary care setting in rural areas. In this model, the primary care provider remains the driver of the mother’s care with the assistance of these specialists.

The MOTHeRS Project aids in patient care by providing appropriate services to women, screening for food insecurity, and linking patients to resources in their community.
Making a Difference
One of the ways that the MOTHeRS Project is serving the community is through screening for food insecurity. In the first 6 months of the program, the MOTHeRS Project has distributed over 100 food bags to clinic sites for patients who identified as food insecure.
